HOW DO I PREPARE FOR MY VISIT?

In order to provide the best possible evaluation we ask that prior to your visit you complete the consultation form available here.


Additionally, we kindly ask that you send over any past medical records pertaining to your current problem (if available) including​

  • doctors' notes

  • lab results if available

  • radiology reports

  • CD of radiology images including x-ray, MRI, and/or CT scans (please request from the performing facility and bring with you to your visit)

WHAT CAN I EXPECT AT MY VISIT?

  • No wait times

  • Comprehensive physical examination

  • Referral for imaging if needed

  • Personalized treatment recommendations that can include a customized exercise program, physical modalities, prescription medications, and referral for specialists

WILL MY INSURANCE COVER MY VISIT?

We provide direct access to care by eliminating the middle man (the insurance company) that often dictates what services can and cannot be provided. Instead we provide the best possible care for our patients as we deem medically necessary. We are re-establishing the doctor-patient relationship the way it was meant to be.

As a result, we are out of network with all insurance companies. A superbill will be provided upon request.

WHAT IS THE CONSULTATION FEE?

The cost for a consultation with Dr. Vijapura is $100.

Payments are payable on the day of service, prior to the appointment.
